If you followed the Nutriforme program and lost weight, we would like to publish your success story

Did you choose NUTRIFORME products to lose weight? Have you lost 25 or 35 pounds or more? We would like to use your success story in our NUTRIFORME promotional campaign on TV, in magazines and in our brochures. Your weight loss should be real and visible. You must include photos taken before and after your weight loss.
If you are interested in sharing your story, please fill out the form below and mail it to us along with your before and after photos. You can also email the form with attached photos. Your success story will not be made public without your prior consent.
Should you meet all the above-mentioned conditions, we will contact you. If your success story appears in our advertising campaign, you will receive appropriate compensation.
